Śivapurāṇa (शिवपुराण) or Śivamahāpurāṇa (शिवमहापुराण).

आद्यन्तमंगलमजातसमानभावमार्यं तमीशमजरामरमात्मदेवम् ।
पंचाननं प्रबलपंचविनोदशीलं संभावये मनसिशंकरमम्बिकेशम् ॥
ādyantamaṃgalamajātasamānabhāvamāryaṃ tamīśamajarāmaramātmadevam।
paṃcānanaṃ prabalapaṃcavinodaśīlaṃ saṃbhāvaye manasiśaṃkaramambikeśam॥
"I meditate on Śiva, the lord of Ambikā, the auspicious one from beginning to end, having no one parallel to him, the noble lord, who is unaging and undying, the lord of souls, the five-faced lord, who is the dispeller of the five powerful sins."
वागर्थाविव सम्प्रुक्तौ वागर्थप्रतिपत्तये।
जगतः पितरौ वन्दे पार्वतीपरमेश्वरौ॥
vāgarthāviva sampruktau vāgarthapratipattaye।
jagataḥ pitarau vande pārvatīparameśvarau॥
I worship Parvati and Parameshwara, the parents of the world, who are inseparable, just like a word and its meaning.
